Care Continuity is a quality improvement system

Your facility already has policies, procedures, clinical standards, and a QAPI program.

Care Continuity works within that environment by examining what happens between the standard and day-to-day execution.

We assess how resident information, responsibilities, handoffs, follow-ups, escalation pathways, and operational knowledge move through your facility. We identify where continuity depends on memory, individual employees, or informal workarounds, and build systems designed to make those processes more reliable across staffing changes.

Then we measure whether the intervention actually improved the identified continuity gap.

The 90-Day Care Continuity Pilot
